First Visit Information

We are so happy that you chose or were referred to Bella Terra Primary Care for your healthcare needs and would like to make your first visit as easy and convenient as possible. Below is what to expect and items to bring on your first visit and what to expect on subsequent return visits. Please review the information as prior patients have shared with our staff how this description made early visits stress free.

When to Arrive:

Please arrive 15 minutes early for your initial appointment in order to complete new patient registration.

Checking In:

When you first arrive at our office for your visit with Dr. Karki, please check in at the reception desk.

Please bring the following items with you (as applicable):

  • Health Insurance card
  • List of any current medications
  • Photo Identification
  • Any referral form(s) the patient has received from primary care physician, ER, etc.
  • Any imaging (X-rays, MRIs, CT scans, etc.) you have had for this condition. All films need to be hand carried to your appointment.

Before your appointment, our staff will make every effort to ensure Dr. Karki is in network with your insurance plan. Our Billing Service will work directly with your insurance to process claims.

If Dr. Karki is not in network with your insurance and you elect to see her on an out-of-network basis, please keep in mind that we are unable to bill third party insurances. You will need to remit full payment at the time of Dr. Karki’s services and then work directly with your insurance company to obtain reimbursement according to your insurance plan.

New Patients and ER Referrals:

If you have been referred to us through the ER or have not previously been seen by Dr. Karki, you will need to read, complete, and submit the following information when you arrive:

Co-Pays and Other Payments:

Your co-payment is required at the time of service.

We accept Visa, MasterCard, Cash, and Checks.

Established patients will be required to keep current all payments that you are responsible for according to your insurance plans.
